The Hidden Impact of Hearing Loss
On-site Training

Five days of training (40 hours) is involved in the process, with the first three days being completed consecutively. This will be followed by:

observation of an individual from THE KOOSER PROGRAM staff teaching the first workshop in your geographic location,
you teaching three workshops independently in your geographic location,
observation by Cathy Kooser-Fathergill of you facilitating a workshop.

TRAINING ACCOMMODATIONS

(Real-time captioning will be provided for those with a hearing loss. If you have any other special needs, please list:

THE KOOSER PROGRAM will be providing a Continental Breakfast and Lunch each day of the training. Please list any special dietary needs or requests you may have. You will be on your own for dinner each evening. There are numerous restaurants around the hotel. (The hotel offers a complimentary shuttle service that will transport you anywhere within a 5 mile radius of the hotel).

TRAVEL AND HOTEL ACCOMMODATIONS

These expenses are the responsibility of the trainee.

Travel Information

We recommend that you arrive at the Dayton International Airport which is approximately 30-40 minutes from our office. Surrounding airports are in Columbus, Ohio (approximately 1½ -1¾ hours away) and Cincinnati, Ohio (approximately 1¼ -1 ½ hours away). You will be responsible for your own transportation to and from the airport.

Taxi cabs will be approximately $40-$50 one way from the Dayton airport and are always available outside of the airport.

Charter Vans Inc. located at the Dayton airport in the baggage claim area by carousel #1 is $35 plus tax. If more than one person shares the van, rates are cheaper. Charter Vans Inc. – 937-898-4043.

On your return flight, please allow 2 hours for check-in at all airports.

Hotel Accommodations

We have arranged a preferred rate with Holiday Inn Dayton Mall in the Club Prestige program. The preferred rate through 2007 is $89 per room per night, plus 13.5% tax. (Rooms are regularly $105-$129).

Your reservation in the Club Prestige Program includes:

Full breakfast buffet featuring “Made to Order Omelets, Waffles and French Toast”, ($9.99 value) for $5.00/person.
Complimentary drinks and appetizers in the Priority Club Room Monday through Thursday from 5:00 PM – 7:00 PM.
Welcome Gift upon arrival.
Priority Club points to the individual paying the charges.

Check in: 4:00PM, Check out: 12:00 Noon.

Cancellation policy is by 6:00PM on day of arrival.

THE KOOSER PROGRAM personnel will make shuttle arrangements with Holiday Inn to transport you to and from the training session each day. We are 3.3 miles from the hotel. The shuttle will leave the hotel each day at 8:15AM to allow for check-in and continental breakfast, and return at 5:00PM on Thursday and Friday to return to the hotel. Training on Saturday will end at 4:00PM.

Depending on your return flight time, you may arrange your transportation to the airport from our office after completion of the training on Saturday.
